What am I being asked to sign?

By signing this form, I am hereby giving my consent to the Mater Foundation to use content from interviews or photographs resulting from photos submitted or a photo shoot, and any reproductions or adaptations of the photographs or words for all general purposes in relation to the Mater Foundation.

I understand that this photograph / video / interview can be used for up to two years.

All personal data that the Mater Foundation use will be collected and stored in accordance with the provisions of the EU regulation 2016/679 General Data Protection Regulation and your rights under GDPR. Please also refer to our Privacy Statement

This consent form is to be completed by the featured individual before content is published, or by parents/guardians if the subject is less than 18 years of age.
